We are now collecting application forms and fees to turn in to the state of North Carolina. The fee is $30 for a specialized plate or $60 for a personalized plate. Regardless of your choice, $20 goes directly to fund cutting-edge ALS research and the balance goes to the DMV to help pay for the North Carolina Wildflower Program. If you would like a “Cure ALS” license plate, please complete the following steps:
1. Download and complete an application form and send it to The ALS Association North Carolina Chapter, Attn: Chandra Green, 4 N. Blount St., Suite 200, Raleigh, NC 27601 or scan and email it to email@example.com.
2. If paying by check, please attach your check made payable to The ALS Association NC Chapter and mail it with your application.

The state approves new specialty license plates once a year. Let’s show them we’re driven to find a cure when they review these applications early in 2018. Once approved, the new license plates will be issued at the time of your renewal.
