This survey will take approximately 10 minutes to complete: 2019 Care Services Survey
The Care Services Department at the North Carolina Chapter is interested in receiving feedback to assess and improve our programs and service delivery.
The ALS Association North Carolina Chapter currently has a total staff of 14 that covers the entire state of North Carolina, serving approximately 525 North Carolinians with ALS at any one time. The Care Services & Advocacy Department attends 11-15 clinic days per month and offers 15 Connection Groups among many other responsibilities. We do a lot with a small staff! See our 2018 By the Numbers here: 2018 By the Numbers
Your input matters! Based on survey results from last year, we have added these services in 2018 and early 2019
