EVENT RULES & REGULATIONS
The Confirmation Card holder does not need to be present to win. Winners shall be notified within 48 hours of the ALS Catfish Classic Golf Tornament which takes place on Monday November 10, 2014, at 5 p.m. The funds raised will be used to help patients and their families cope with ALS through patient programs, financial grants for respite care, transportation, and augmentative communication, as well as access to a durable medical equipment loan closet, research and patient support groups.

HOW TO PLAY: Conditions & Limitations
Winners will be notified by phone or email. All federal, state and local taxes are the sole responsibility of the individual winner. The IRS Tax ID Number is 56-1609591 and documentation can be provided upon request.
